TO PAY BY CHEQUE Please send a cheque for the required amount made payable to TheDoctor Ltd (Note: TheDoctor is one word, no spaces between The and Doctor). Write your name, d.o.b, male or female, address, email and telephone number including which blood tests you have ordered on a separate piece of paper, along with your cheque.
